You will focus on chronic disease management and preventative care, contributing to quality improvement and mentorship for learners. This role ensures you make impactful connections within diverse populations while enjoying a healthy work-life balance.
Key Responsibilities:
• Deliver evidence-based primary healthcare services
• Support chronic disease management and health promotion




• Participate in interprofessional case conferencing
• Mentor and teach learners when applicable
• Engage in quality improvement initiatives
Requirements:
• Licensed with CPSO for independent practice
• Certification in Family Medicine preferred
• Strong commitment to team-based care
• Interest in community health and preventive care
• Completed Vulnerable Sector Police Check
