Detailed overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Registered Nurse provides direct patient care by assessing, planning, providing and evaluating nursing care to patients, in collaboration with the interprofessional care team for person-centered care.

The purpose of this position is to provide qualified registered nurses the opportunity to consolidate their skills by rotating through a specialty nursing unit(s) prior to and/or during post graduate coursework applicable to the relevant specialty nursing unit(s). Successful applicants are expected to participate in and complete the applicable post graduate specialty nursing courses/program within agreed timelines.

Responsibilities

- Assesses patients and collects information from a variety of sources using observation, communication, data review and physical assessment. Diagnoses the patient’s condition and determines appropriate nursing interventions. Plans,



performs and evaluates nursing interventions and care.
- Collaborates with patients and families/caregivers in developing the care plan and encourages family participation. Establishes a therapeutic relationship, demonstrates empathy, trust and respect, and advocates for privacy, dignity and patient autonomy in decision making.
- Participates in care and discharge planning to ensure an effective plan is established, implemented and evaluated; updates care plans promptly and collaborates with the interprofessional care team and the Patient Care Coordinator on discharge-related issues.
- Coordinates nursing care for the patient; sets priorities, assigns and delegates tasks to support staff, and makes referrals to the interprofessional care team according to established protocols and standards.
- Collaborates with physicians, Licensed Practical Nurses, Patient Care Coordinators, Clinical Nurse Educators and other providers to identify issues, adjust care plans as needed, and respond to changes in patient condition.
- Teaches patients and families about illness prevention, health restoration and health maintenance; assesses learning needs and provides information and demonstrations to support self-care and discharge planning.
- Plans and discusses referral information with patients and families; initiates referrals to appropriate health care or community agencies in collaboration with the care team.
- Documents assessments, nursing diagnoses, observations, interventions, patient/family responses, outcomes/evaluations, and referrals according to practice standards and Fraser Health procedures.
- Participates on nursing committees, care conferences and councils as assigned, and engages in research and quality improvement activities by identifying issues and recommending changes to practice.
- Attends in-services and other educational programs; maintains current clinical competence and knowledge in area of practice.




- Provides feedback to the Manager regarding the effectiveness of patient care programs, policy/clinical decision support tools and equipment.
- Performs other related duties as required.

Qualifications

Education and Experience

Graduation from an approved School of Nursing with current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

Completion of the applicable post graduate specialty nursing course/program and/or willingness to complete the applicable post graduate specialty nursing courses/program.

Current certification in Basic Cardiac Life Support (BCLS).
