Support seniors and their families in exploring private senior residences and home care options. Provide personalized support based on attentive listening, understanding of needs, and trust. Facilitate transitions to a current living environment with professionalism and empathy. Develop lasting relationships with partners (healthcare professionals, community organizations, and local stakeholders). Ensure diligent follow-up and maintain organized case and file management.

- Ability to support and guide vulnerable individuals in collaboration with their families or support network
- Excellent communication and interpersonal skills
- Experience in customer service, consultative sales, or business development
- Self-reliance, initiative, and an entrepreneurial mindset
- Valid driver's license and a vehicle
- Experience in the healthcare field or working with a senior population is considered an asset
