The Assertive Community Transition Team (ACTT-SW) at CMHA is seeking a Registered Practical Nurse or Registered Nurse. This role is crucial for supporting clients aged 16 and older, facing challenges due to serious mental illness. With a focus on recovery and empowerment, you will manage medication, conduct assessments, and coordinate treatment plans while collaborating with the ACTT.
Key Responsibilities:
• Conduct mental and physical health assessments
• Manage and administer medication per protocols
• Provide treatment, support, and rehabilitation
• Develop personalized treatment plans for clients
• Collaborate closely with the ACTT psychiatrist
Requirements:
• Licensed Registered Practical Nurse or Registered Nurse
• At least four years of experience in mental health
• Strong health assessment and clinical judgment skills
• Knowledge of relevant mental health legislation
• Valid Canadian driver’s license and insurance
