About the Role

Reporting to the Clinic Manager, the Registered Social Worker collaborates with our Mental Health Team to provide front-line support to individuals and families accessing primary care services at IIPCT.



This position combines culturally responsive practice with evidence-based social work to support individuals through a wide range of social, emotional, and mental health needs.

What You'll Do

Provide social work services to individuals and families accessing Primary Care Team services, including: Needs assessments

Counselling and crisis intervention

Advocacy and referrals

Assistance with forms and system navigation

Empower FNIM individuals and families by delivering culturally protected, strengths-based care that promotes wellness and resilience.

Collaborate closely with Mental Health Educators, the Mental Health Team, physicians, and other internal programs to provide coordinated care.

Maintain an active caseload while completing documentation, statistical reporting, and other administrative responsibilities.

Participate in team meetings, case conferences, and organizational initiatives.

Support additional projects and responsibilities as assigned.

What You'll Bring

Registered Social Worker (RSW) in good standing with a Master's degree in Social Work or a minimum of three years of direct social work experience.

Experience working with Indigenous individuals, families, and communities,



with an understanding of culturally grounded approaches to care.

Knowledge of First Nations, Inuit, and Métis cultures, values, and histories.

Experience supporting clients with challenges related to: Mental health

Poverty and housing

Family violence

Legal and social systems

Substance use

Training or experience in crisis intervention, trauma-informed care, counselling, mental health assessment, substance use screening, and supporting people living with disabilities.

Fluency in a local Indigenous language is considered an asset.

Strong computer skills.

Willingness to travel between IIPCT sites and throughout the Deseronto area as required.

Valid Ontario driver's licence and access to a reliable vehicle.

Current immunizations and a satisfactory police record check.

Commitment to participating in Indigenous Cultural Safety training and cultural teachings.
