Delta Division of Family Practice — Lead the Transformation of Primary Care in South Delta
Are you a strategic and collaborative health care leader with a passion for enhancing community-based primary care? This exciting prospect serves as the Primary Care Network (PCN) Director/Manager in South Delta. Reporting to the Executive Director, you will be responsible for operationalizing the Ministry of Health's PCN initiative within the region. Working closely with the local Division of Family Practice, health authority, First Nations communities, and other key stakeholders, you will oversee the day-to-day implementation of the PCN Service Plan.
Responsibilities
As the PCN Director/Manager, you will:
Leverage solid project management, communication, and facilitation skills to drive team-oriented relationships.
Foster community engagement and champion the principles of primary health care, population health, and health promotion.
Lead the transformation of primary care in South Delta with a focus on integrated service delivery.
Operationalize the PCN Service Plan in collaboration with stakeholders.
Qualifications
To succeed in this role, you will need:
A minimum of 5 years' experience in a distributed leadership capacity within the health sector.
Expertise in quality improvement, change management, and cultural safety.
A post-secondary degree in a health-related discipline is required, with a master's degree preferred.
