Vancouver Coastal Health (VCH) is seeking a Social Worker to address client and family-centred care needs within an integrated health service team. You will work in accordance with VCH policies and a trauma-informed, recovery-oriented model of care across sites, clinics and services.
The role includes comprehensive assessments, care coordination, and collaboration with diverse health professionals to support transitions through the continuum of services for complex discharge and intake needs.
