Sunnybrook Health Sciences Centre in Toronto is seeking a Registered Practical Nurse to provide person-centred care in an acute care setting within the C6 Community Programme. You will work within an interprofessional team to support Internal Medicine, Gynecology Oncology and Plastic Surgery patients with complex needs.
Responsibilities include assessments, planning, implementing evidence-based interventions, and contributing to quality improvement and discharge planning.
