Position: Private Duty Nurse (RPN or RN)

We are seeking a compassionate, experienced, and dependable Registered Practical Nurse (RPN) or Registered Nurse (RN) to provide one-on-one nursing care for a medically fragile 19-year-old in a home setting.

Client has a tracheostomy and is always on mechanical ventilation. He uses an eye gaze computer and facial expressions for communication and is very active in his community. He is a 2 person transfer and has a PSW to assist nurse in the home with care. All training will be done by the home care team and/or parents. No behavioral issues. Friendly, helpful home care team. Independent contract through FMHC (family managed home care).

The ideal candidate will have experience caring for individuals with complex medical needs, including tracheostomy and mechanical ventilation, and be committed to delivering safe, high-quality, family-centered care that promotes comfort, dignity, and quality of life.

Responsibilities

- Provide skilled nursing care in accordance with the client's individualized care plan.
- Monitor and manage a tracheostomy and mechanical ventilator.
- Perform tracheostomy care, suctioning, and airway management as required.
- Assess respiratory status, vital signs, oxygen saturation, and overall health status.
- Administer medications, treatments, and enteral feedings as prescribed.
- Recognize changes in the client's condition and respond appropriately to emergencies.
- Operate and troubleshoot medical equipment, including ventilators, suction machines, pulse oximeters, and oxygen therapy equipment.




- Maintain accurate nursing documentation and communicate changes in condition to the family and healthcare team.
- Implement infection prevention and control practices.
- Assist with personal care, positioning, transfers, and activities of daily living as needed.
- Support the client in participating safely in home and community activities when appropriate.
- Maintain a safe, respectful, and supportive home environment.

Qualifications

- Current registration in positive standing with the College of Nurses of Ontario (CNO) as an RN or RPN.
- Current BLS (Basic Life Support/CPR) certification.
- Vulnerable Sector Check
- Liability Insurance
- Experience with tracheostomy care and mechanical ventilation.
- Experience providing home care or pediatric/young adult complex care is considered an asset.
- Strong clinical assessment, critical thinking, and emergency response skills.
- Excellent communication and interpersonal skills.
- Ability to work independently while collaborating effectively with the family and healthcare team.

Preferred Qualifications

- Experience with enteral feeding, medication administration, and respiratory care.
- Knowledge of home ventilator systems and emergency airway management.
- Experience supporting medically fragile adolescents or young adults.

Schedule

- Part-time position available
- Evenings, weekends, and occasional Fridays, plus covering for other nurses.
