1.0 FTE (37.5 hours per week)- Dufferin-Caledon

The Dufferin Area Family Health Team (DAFHT) is currently seeking a full-time (1.0 FTE or 37.5 hours per week) Occupational Therapist in Dufferin-Caledon . The Occupational Therapist (OT) is a vital part of the DAFHT and collaborates with the physicians and other interdisciplinary providers to achieve the mission and vision of the organization. The OT helps the DAFHT to meet its strategic objectives by participating in the chronic disease management, health promotion and disease prevention programs.

Duties and Responsibilities:
Provide Occupational Therapy assessment, treatment planning and intervention for adult patients ages 18+ with a focus on senior’s health, chronic disease management, and functional independence

Contribute to an interdisciplinary memory clinic one day per week, including participation in cognitive and functional assessments, care planning, patient and caregiver education, and team-based management of patients experiencing cognitive decline

Support patients with falls prevention strategies through assessment, education and individualized intervention plans

Conduct mobility assessments and recommend adaptive equipment, assistive devices and environmental adaptations to improve safety and independence

Address home safety and provide recommendations to patients and family members, to reduce risk and support aging in place

Assist patients with return to function and return to work planning following injury, illness or change in health status

Support individuals with chronic pain, insomnia and other chronic health conditions through education, self-management strategies, pacing, energy conservation and coping techniques

Collaborate with patients to develop functional goals to enhance participation in daily activities, leisure, work and community life

Develop and facilitate therapeutic and educational groups related to chronic disease management, healthy aging and wellness

Provide patient and caregiver education regarding health management, safety, and community resources

Complete functional assessments to evaluate patients’ physical,



cognitive and psychosocial abilities in order to support physicians with medical documentation and completion of forms related to disability claims, benefit eligibility and other reporting requirements

Participate in quality improvement initiatives, program development and health promotion activities within the Family Health Team.

Skills and Abilities:
Strong clinical assessment and problem-solving skills related to physical, cognitive, psychosocial and functional abilities

Intermediate experience and strength in applying motivational interviewing counselling techniques in an adult population

Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery, with an emphasis on seniors’ health and chronic disease self-management

Experience and enthusiasm in facilitating group educational and therapeutic sessions using established adult teaching/learning theory principals

Experience completing functional ability evaluations and providing objective clinical information to support disability, benefits and return to work documentation. Knowledgeable about government funding/programs and application assessment and application process (e.g., ODSP, Disability Tax Credit, Assistive Device Programs) is considered an asset

An understanding of and critical analysis of the determinants of health and social factors

Excellent verbal and written communication skills, with the ability to clearly convey information and ideas

Ability and enthusiasm to work effectively with all members of the health care team

Willingness to teach and inform other members of the interdisciplinary team or students about the role of OT, as required

Strong Leadership, organization, research, evaluation, time management, communication, and interpersonal skills

Experience with an Electronic Medical Record (EMR) is beneficial

Knowledge of the goals, structure, policies and procedures,



and programs of a family health team setting.

Education/Experience:
MSc in Occupational Therapy required

Current registration in good standing with the College of Occupational Therapists of Ontario required

Member of the Canadian Association of Occupational Therapists (CAOT) &/or Ontario Society of Occupational Therapist (OSOT) preferred

Minimum two years’ experience, with a passion for primary health care (some PHC experience preferred)

Experience working with seniors, chronic pain, return-to-work programs and group facilitation is considered an asset

ADP authorizer status considered an asset.

Working Conditions and Requirements:
Hourly Rate: $38.79

This is a hybrid role

Office location is Orangeville-Caledon

Hours are 37.5 hours per week – including one evening shift per week

Vehicle for travel to sites and within community.
