We are seeking a compassionate and skilled Wound Care/Catheter Care Nurse, either Licensed Practical Nurse (LPN) or Registered Nurse (RN), to join our dedicated healthcare team. In this vital role, you will provide specialized wound management and catheter care to diverse patient populations, including those in nursing homes, and home care settings. Your expertise will ensure optimal healing, comfort, and safety for patients with complex wound and catheter needs. Responsibilities

- Assess patients’ wounds and catheter sites regularly to monitor healing progress and identify complications early.
- Implement evidence-based wound care protocols, including dressing changes, debridement, and infection control measures. (With Doctor's Order)
- Administer medications accurately, including topical agents and pain management therapies related to wound and catheter care.
- Educate patients and their families, as well as care staff on wound management techniques, catheter maintenance, and infection prevention strategies.
- Collaborate with interdisciplinary teams to develop individualized care plans that address complex medical needs such as dementia care, hospice requirements, or pediatric considerations.




- Document all patient interactions thoroughly using electronic health record systems; maintain detailed records of treatments and progress.
- Assist with laboratory procedures such as phlebotomy when needed, ensuring proper specimen collection for diagnostic testing.

Qualifications

- Valid LPN or RN license in good standing; prior experience in nursing home, homecare, hospital, or acute care settings strongly preferred.
- Demonstrated expertise in wound care management, including experience with advanced dressings and infection control practices.
- Knowledge of medical terminology, medication administration protocols, and patient safety standards.
- Familiarity with electronic health record systems such as Epic or Cerner; experience with medical office procedures is a plus.
- Experience working with diverse patient populations including seniors, children, dementia patients, or hospice clients is highly desirable.
- Strong communication skills with the ability to educate patients and collaborate effectively within multidisciplinary teams.
- Additional skills that enhance your candidacy include assisted living experience, pain management expertise, case management background, laboratory experience, or familiarity with home care environments.

Pay: $32.00-$36.00 per hour
