Position Title: Nurse Practitioner (NP) - GAIN Community
Employment Status: Temporary Full-time Contract until September 2026 (Maternity Leave Coverage)
Position Status: This is a Bargaining Unit position. All terms and conditions of the collective bargaining agreement apply.

Salary Range: Annualized salary range $121,119.00 - $125,916.00 commensurate on skills and experience, plus participation in HOOPP.
Hours of Work/Schedule: 35 hours a week, Monday to Friday 8:30am - 4:30pm, includes home visits
Position Reports to: Senior Manager, Integrated Care and Experience
Primary Location: Oshawa (with flexibility to transition to/work at other DCHC sites as required).
Organization: Durham Community Health Centre (DCHC) is a registered, charitable organization that provides integrated, accessible, and equitable community-based primary care, wellness services, and health education to Durham community members who face multiple barriers to their health and well-being.
Position Overview: As part of the Chronic Disease Management, Clinical Services Team, this Nurse Practitioner will primarily care for elderly or frail clients referred to the GAIN Community Team. The role focuses on conducting client assessments, providing direct support and expertise during community home visits and in-office clinical visits, and leading the provision of comprehensive geriatric services.
Key Responsibilities
Clinical Practice
- Utilises a comprehensive theoretical knowledge base and advanced clinical competence in caring for acute medical and continuing care clients, collecting and interpreting health data to determine diagnoses and treat complex problems with support from collaborating physicians.
- Provides comprehensive geriatric assessments, including interviews, physical examinations, psychosocial, cultural, and ethnic factors, ordering diagnostic tests and procedures, and reviewing client health data.
- Delivers direct patient care focused on health promotion and maximizing safety and function for frail seniors living at home.
- Develops and implements goals and treatment interventions for complex cases to inform an integrative care plan with the GAIN Community Team.
- Implements the comprehensive care plan, prescribes medications, orders interventions, and coordinates with team members to facilitate productive movement through the healthcare system.
- Monitors clients' goals and outcomes and initiates planning processes.
- Participates in case presentation, discussion, and review in a team format.
- Provides outreach assessment and case management services in program catchment areas.
- Prepares comprehensive reports to referral parties and maintains documentation up to discharge.
Administration
- Initiates and participates in program planning activities within the GAIN team under the manager's direction.
- Participates in formal and informal program evaluation to monitor client needs for ongoing planning.
- Engages in team consultation with colleagues.
- Liaises with relevant healthcare/hospital services and the community as a consultant/resource.
- Ensures program development aligns with the Agency's mission, vision, and values.
- Maintains professional development relevant to delivering NP services.
- Maintains statistical records related to practice, recording workload statistics per program policies.
- Maintains flex time to accommodate department needs.
- Participates in committees.
Other Duties Within Scope
Consult/Knowledge Transfer/Support
- Assists in orientation and training of new GAIN staff.
- Designs and implements strategies to train staff from other disciplines on functional assessments.
- Collaborates with GAIN colleagues in establishing goals and their delivery.
- Maintains knowledge and information level conducive to competent functioning in assignment areas.
- Consults regularly with team members concerning psychosocial management of clients.
- Provides advice and information on frail elderly client concerns.
- Consults with local agencies or community resources for resource development and program planning.
Key Qualifications
- Master's Degree in Nursing (MSN) with completion of a recognised primary care NP program (NP-PHC).
- Current Certificate of Competence with the College of Nurses of Ontario (RN Extended Class); geriatric training and certificates such as P.I.E.C.E.S., Montessori, and G.P.A assets.
- Familiarity with Comprehensive Geriatric Assessment and related tools.
- Three years' experience as a primary care NP with clients having complex needs, including geriatric population and older adults.
- Skills and experience to fulfill major functions of the role including wellness care, assessment, and diagnosis.
- Proven ability to coordinate care collaboratively with interdisciplinary team members and function independently with clients.
- Demonstrated ability to relate therapeutically with clients, including strong negotiation and conflict resolution skills.
- Facilitation skills for education and promotion of health programs.
- Demonstrated leadership in advancing clinical practice and achieving program goals.
- Demonstrated use of theory and research/evidence-based outcomes in practice.
- Valid Ontario Driver's license and access to a vehicle.
