Key Responsibilities:
• Provide direct personal support to patients daily
• Aid patients with grooming, bathing, and feeding tasks

Requirements:
• Completion of a PSW or equivalent certification
• Prior experience in community or long-term care desired
• Knowledge of care planning systems
• Strong command of English, both written and verbal
• Transportation access for patient home care support
