Key Responsibilities:
Implement and evaluate resident care/service plans
Facilitate communication and coordination with healthcare providers
Manage medication orders and document compliance




Conduct follow-ups on resident/family interactions
Collaborate on Infection Prevention and Control initiatives

Requirements:
Active registration as a Registered Practical Nurse
Experience with unregulated care providers preferred
Robust oral and written communication skills
Basic Cardiac Life Support certification necessary
Recent Vulnerable Position/Sector Screening required
