Key Responsibilities:
• Provide case management services and develop Case Plans
• Undertake needs assessments and assist with mental health issues
• Advocate for clients navigating healthcare appointments




• Collaborate with the multidisciplinary team on strategies
• Monitor each resident's housing challenges and adherence to policies
Requirements:
• Bachelor’s degree in Social Work or related field
