This role focuses on delivering person-and family-centered care to support those with diverse backgrounds. As a critical member of the multi-disciplinary team, you will implement behavioral interventions, teach activities of daily living (ADLs), and collaborate on individualized care plans. Your input will significantly impact patients’ transitions into and out of community programs.
Key Responsibilities:
• Implement behavioral and rehabilitation interventions for community patients
• Teach ADLs and apply relevant OT, PT, and SLP strategies




• Provide routine care and escort clients to appointments
• Conduct assessments and document patient progress
• Collaborate with healthcare specialists for comprehensive patient support
Requirements:
• Minimum 2 years experience in behavior assessments
• Post-secondary education in behavior sciences or psychology
• Knowledge of psychogeriatric assessment tools
• Solid communication and problem-solving skills
• Ability to manage intense emotional situations
