You will assist individuals in building life skills and relationships while participating in various recreational, social, and community-based activities. Collaborating with families and professionals, you will develop and monitor individualized support plans tailored to each person's needs.
Key Responsibilities:
• Build relationships to support personal goal achievement
• Develop and implement individualized support plans
• Assist with personal care and daily living activities
• Support protected food preparation and handling practices
• Transport individuals to community activities as required
Requirements:
• Diploma or Certificate in Community or School Support Work
• Minimum one year of related experience required
• Valid Class 4 Unrestricted Driver's License
• Ability to lift and assist individuals safely
• Criminal record check with vulnerable sector search
