Job description:

The Social Worker - BSW Senior, reporting to the Manager or their designate, will coordinate care for patients, clients and residents. The incumbent will provide case management, education, advocacy, urgent consultations, referrals, and consultation to appropriate services and provides in office and virtual social work services.

Experience
2 years SW experience working with issues of capacity, dementia, grief, and loss
Two years experience with social work practice in a medical setting
