Key Responsibilities:
• Build positive relationships and provide emotional support
• Deliver one-on-one assistance per Coordinated Care Plans
• Participate in participant intake and screening processes




• Monitor client progress and well-being effectively
• Support clients in goal setting for independence
Requirements:
• Social Service Worker diploma or related education
• Over 2 years' experience with older adults and vulnerable populations
• Solid de-escalation and crisis intervention skills
• Proficient written and oral communication abilities
• Competency in utilizing technology and community resources
