Description

Hours of Work : Mon.-Fri. 8:30a-3:30p (70 hrs. biweekly) (CSWII - Club House 55)

The Job Responsibilities are as follows:
Supporting our clients in their facility and community based activities and arranging transportation when required.
Participating in client care meetings with the clients and their circle of supports to discuss progress and/or concerns.
Assisting clients with their support plan goals, which contain measurable outcomes and reflect the client’s interests, skills and choices in recreation and leisure, learning, cultural, spiritual interests and/or hobbies. Providing direct and indirect client support while on site and out in the community with our clients.
Providing direct support to clients ensuring their safety,



and supporting clients with opportunities to grow in their skill sets and independence while advocating for their fair and protected access to all opportunities.
Identifying community resources available to the client and facilitate client access to those resources.
Monitoring progress and reporting progress and issues to the Service Coordinator for discussion and problem solving.
Completing and maintaining the necessary documentation, administrative and reporting processes for the client’s master file, and other reporting procedures as required.
Providing support to other teams on an as needed basis.
Providing personal care to clients as required.

Requirements:
Satisfactory current criminal record check with vulnerable sector is mandate.
Maintain certification in First Aid & CPR Level C is mandate.
