Job Summary

We are seeking a compassionate Nurse Practitioner specializing in primary care to cover 1 day a week for a 6 hour shift, including a lunch break.

In this vital role, you will provide comprehensive, patient-centered medical services across a diverse patient population, including children, seniors, and individuals requiring acute or chronic care management. This position offers an exciting opportunity to make a meaningful difference in patients’ lives through proactive health management and personalized care.

Responsibilities

- Conduct thorough patient assessments, including history-taking and physical examinations for individuals of all ages, with special attention to pediatrics and senior care.
- Develop and implement personalized treatment plans that incorporate medication management, pain control strategies, and health education.
- use AVAROS EMR




- Collaborate with multidisciplinary teams to ensure seamless case management and continuity of care across various healthcare settings.
- Educate patients and families on health promotion, disease prevention, medication usage, and lifestyle modifications to enhance overall well-being.
- Maintain detailed medical records adhering to regulatory standards while utilizing electronic health record systems efficiently.

Experience
