Bayshore HealthCare is seeking a part-time Occupational Therapist for the Ontario home-care ICS team in Kitchener. You will assess, plan, and implement OT services to support older adults transitioning from hospital to home, coordinating with a multidisciplinary team and ensuring quality care in the community.
You will document thoroughly, apply evidence-based interventions, and contribute to care planning and patient outcomes while maintaining qualified standards and provincial regulations.
