As the RAI Coordinator, you will manage and implement the Minimum Data Set for a collaborative care environment. Your expertise in long-term care ensures that interdisciplinary teams receive essential education and support for accurate MDS documentation. This position requires a solid commitment to improving resident assessment practices to uphold standards and timelines.
Key Responsibilities:
• Provide RAI MDS education to interdisciplinary teams
• Monitor on-site MDS documentation and practices
• Maintain an updated resident assessment schedule
• Assist with coding assessments and accuracy support
• Audit completion of MDS assessments to meet timelines
Requirements:
• Three years of clinical or project leadership experience
• Knowledge of Long Term Care Act and standards
• Current RN or RPN registration in Ontario
• Strong background in long-term care practices
• Commitment to resident-centered care initiatives
