What you will do:

Manage a caseload of major equipment and adaptation cases for disabled children and young people, conducting assessments, providing recommendations, and ensuring the successful delivery of services.

Offer professional guidance to peers and multi-agency staff on matters related to disability, helping shape effective decisions and service provisions.

Collaborate with statutory agencies, housing providers, District Councils, health commissioners, provider trusts, and voluntary organisations to ensure seamless service delivery and optimal outcomes for clients.

Qualifications / Requirements:

HCPC registered.

Valid driving licence, as travel within Sutton may be necessary to effectively serve clients.
