Prime Care Family Health Team, is an interdisciplinary healthcare team, focused on primary care in collaboration with family physician affiliates. Our patient population includes; patients enrolled with our physician affiliates and unattached community patients.

JOB description:

As a member of the interdisciplinary health care team, including a nurse practitioner team, the nurse practitioner will provide patients with full-scope primary care.

Required:

- Registered with the of College of Nurses of Ontario as a Nurse Practitioner (RNEC) - in good standing and without practice restrictions
- MINIMUM of 3 years of experience in a primary care setting ( FHT , CHC, NPLC)
- A current “Clear Vulnerable Sector Screening Program – Police Reference Check”, as a condition of employment

JOB details:

· Permanent Position

· 1.0 FTE ( 40 hours per week daily 60-minute lunch period)

· 5 days per week

· Annual Salary for 1.0 FTE ( $ 125, 477.00 ) - less all applicable source deductions)

· In office, Milton Ontario





· Anticipated start date Tuesday August 01/ 2026.

· HOOPP and Extended Health Benefits upon completion of 3-month probationary period.

Reporting:

The Nurse Practitioner shall collaborate with the Prime Care Led Nurse, the Prime Care Operations and Human Resources Directors- as appropriate and will ultimately report to the Prime Care Executive Director.
