Job Overview
Position: Community Support Worker Lead

Work Location: Lakeshore/DeHart

We are hiring an experienced Support Lead for a specialized, high-support program.

Regular schedule:

- Thursday–Saturday: Awake Overnight, 22:30–06:30
- 2 AM/PM shifts

You must also be flexible and available to work all 3 shifts and pick up additional shifts when required:

- 06:30–14:30
- 14:30–22:30
- 22:30–06:30

We cannot accommodate modified start/end times, early departures, or individualized schedules.

This is a leadership position supporting an individual with complex physical, cognitive, and medical needs. The Support Lead must be confident working independently and making appropriate decisions without a manager onsite.

You will lead shifts, support the team, monitor the individual's health, follow the care plan, make decisions regarding PRN medication according to protocol, adjust the daily schedule when needed, support de-escalation, and remain calm in high-stress situations.

You will also support community outings, driving, rehabilitation, and trying current activities.

The Supported Individual:

- Active lifestyle and regular community activities, including adaptive hikes and hydrotherapy

The successful Candidate will:

- Have a minimum of 3 years of relevant education and/or work experience
- Independently run a shift and make sound decisions
- Be confident responding to seizures and supporting individuals with cognitive and physical challenges
- Have strong knowledge of infection control and immunocompromised client safety




- Have advanced professional English skills – speaking, reading, and writing
- Hold a full Canadian Class 5 driver's license
- Have their own reliable vehicle and transportation
- Be comfortable driving larger vehicles, including wheelchair SUVs
- Be confident driving in all seasons

Responsibilities:

- Assist patients with daily activities such as bathing, dressing, feeding, and mobility to promote comfort and independence
- Support with individual care tasks including vital signs monitoring and medication administration under supervision
- Communicate effectively with individuals, families, and healthcare team members to ensure clear understanding of care plans
- Follow infection control procedures and safety protocols to maintain a clean and safe environment for individual and staff
- Assist with medical procedures as directed by licensed medical professionals using appropriate medical terminology

Compensation:

3 month probationary wage : $25.95/hour

Following successful completion of 3 months probation, employee may apply for Support Lead position @ 30.28/hour.
