Role Overview

We are looking for a Registered Nurse to join our team. In this role, you will provide patient counselling and education focused on chronic disease management, lifestyle changes, and nutrition support to help patients improve their clinical outcomes.

You will work closely with Registered Dietitians, physicians, and other healthcare professionals to provide patient-centered care and help patients build healthier, sustainable habits.

Responsibilities

- Conduct thorough assessments and review medical and lifestyle history
- Provide education and counselling for chronic disease management, including diabetes, high cholesterol, hypertension, fatty liver, weight management, and general wellness
- Support patients with nutrition, physical activity, and healthy lifestyle changes




- Provide practical guidance to help patients improve their health and build sustainable habits
- Collaborate with healthcare professionals to deliver holistic care
- Document patient care accurately and professionally

Qualifications

- Current registration in good standing with the College of Nurses of Ontario
- Bachelor’s degree in Nursing
- Skills in counselling, patient education, and communication
- Strong interpersonal and communication skills
- Ability to work collaboratively in a team workplace and independently
- Knowledge of nutrition and healthy lifestyle education is considered an asset.
