Family and Client Care Coordinator (RPN) and LEGHO CoordinatorDuties & Responsibilities:Responsibilities to Clients

- Refer clients for completion of RAI-CHA assessments as required
- Review client assessments electronically as required
- Recommends and facilitates appropriate CSS and HCSS referrals that meet client needs
- Perform a Case Management function by actively promoting/linking services of other health system partners and other sectors as appropriate as well as assists clients to navigate the system
- Teach, delegate, assign, and supervise medication administration and other controlled acts permitted under the Regulated Health Professions Act, 1991
- Ensures clients have access to the information they need to participate in their care planning, research, and source out information as required
- Develop a plan of care for clients
- Prepare clients and/or families for discharge

Responsibilities to Family/Caregiver Support

- Provide education, information, and counselling support to the caregivers of clients
- Ensure ongoing communication and support with families/caregivers of clients
- Provide information to family/caregivers regarding other support services in their community

Responsibilities to Administration

- Maintains and updates an accurate electronic calendar of work schedule including appointments, locations, and shares with the Intake Manager
- Ensure client records are maintained and current
- Gather statistics as required by Intake Manager

General Duties

- Self-manages work schedule, coordinating availability in a way that responds to internal requirements
- Networks with and cultivates relationships with relevant community resources and CSS agencies
- Ensures that appropriate information and documents are available to system partners and applicable CSS agencies




- Ensures that client confidentiality is actively and responsibly managed and that client information, applications, files, assessments, electronic or otherwise are maintained in accordance with privacy policies and standards
- Transfers information, assessment data or files within HCSS and to other authorized CSS agencies/systems partners adhering to the prescribed privacy policies and standards
- Coordinates with and works professionally and effectively with CSS agencies and other system partners
- Positively and professionally represents all CSS agencies and service providers in the Ontario Health West region
- Assist in the promotion of the services of HCSS, especially to healthcare professionals
- Seek ongoing education opportunities which will enhance knowledge pertinent to the role, including about HCSS Grey-Bruce services, clients, and the sector
- Maintain privacy and confidentiality
- Adhere to the Occupational Health and Safety Act and its associated regulations
- Complete mandatory annual online training courses

- Experienced in the use of client assessment tools




- Must be available for overnights/evenings
- Excellent communication and problem-solving skills
- Above-average competence with current technology and software
- Strong interpersonal skills and ability to work effectively as a member of a team
- 3-5 years’ experience working in a home and community care organization or social/health services program
- Broad working knowledge of Community Resources
- Must have a valid Driver’s License and access to a reliable vehicle
- Knowledge of Alzheimer's Disease and related disorders
- Previous experience working with volunteers is an asset
- Current registration with the College of Nurses is required
- Travel to other Day Away Programs across Grey-Bruce, with a mileage reimbursement

Compensation & Total Rewards

- Wages are from $27.22/hour, 56 hours biweekly.
- Four days (28 hours biweekly) in LEGHO Coordination
- Four days (28 hours biweekly) in HCSS Adult Day Program, in the Port Elgin site
- 8 days of PTO per annum
- 3 weeks of vacation, per annum (12 days per year)
- Benefits, including dental
- TFSA/RRSP matching program, up to 5%
