- Proficient in care coordination and case management principles
- Excellent communication and interpersonal skills
- Ability to work collaboratively in multidisciplinary team workplace
- Strong organizational and time management skills
- Willingness to become familiar and use Meditech Expanse What You’ll Do - Conduct comprehensive assessments of pediatric patients and their families
- Develop and implement individualized care plans in collaboration with healthcare teams
- Coordinate referrals to specialists, therapists and community resources and monitor follow up reports
- Provide education on nutrition, preventive care and chronic disease management
- Facilitate support groups and community health initiatives




- Document patient interactions and care plans in electronic health records What Your Bring - Bachelor of Science in Nursing
- Registered with College of Nurses of Ontario and in good standing
- Certification in PALS and AED is an asset
- Respect for diverse cultures and traditions
- Valid Drivers License – Class G
