Join NorWest Community Health Centres as a Peer Leader for the CDSMP. This volunteer position helps individuals with chronic conditions gain essential self-management skills. As a Peer Leader, you will facilitate groups and help participants better manage their health.

Ideal candidates will have a strong understanding of chronic diseases and excellent communication abilities, along with an empathetic approach to foster participant engagement. You do not need previous healthcare experience to make a difference. Key Responsibilities:

- Facilitate online group discussions on health management
- Share effective self-management practices and insights
- Encourage participation and provide emotional support
- Assist with the organization of materials and resources
- Tailor discussions to suit participant needs Requirements:
- Good understanding of chronic disease management
- Robust empathy and communication skills
- No healthcare background required
- Commitment to participating in virtual training sessions
- Open to learning and sharing knowledge Make a difference by helping others take control of their health through informed self-management practices.
