Holistic Intake and System Navigation Counsellor (The Corner)

TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals.

The Corner is a multi-service, inclusive hub governed by residents and service providers and is a unique, integrated service model between 20 partner organizations. It is the primary hub for Health Access St.

James

Town (HASJT) intake, referrals and service coordination and is aimed to enhance interdisciplinary care including primary care, improving access to health, social and community services in St.

James

Town.

The Neighbourhood

Organization leads this integrated service model comprising The Corner and HASJT. We are seeking one Holistic Intake and System Navigation Counsellor to join our The Corner team in a full-time permanent role.

The ideal candidate should have a Bachelor of Social Work (BSW) or equivalent education and experience in health or social sciences, along with 3–5 years of experience in a community health setting or a comparable combination. A minimum of 1–3 years of experience working with newcomers and diverse populations—including 2SLGBTQ communities, individuals experiencing homelessness, and those who use substances—is essential. Strong analytical and decision-making skills are required to assess client needs, resolve issues, and develop appropriate service plans, along with proven crisis intervention abilities. Candidates must demonstrate excellent communication skills in English (written and verbal), with fluency in a second community language considered an asset.

Strong organizational, coordination, and interpersonal skills are necessary, as is the ability to manage fluctuating workloads and respond quickly to crises. A strong understanding of harm reduction and anti-oppression frameworks, cultural sensitivity, and experience supporting individuals with mental health and/or addiction challenges are key.



Proficiency in using computers and standard office equipment is also required.

DUTIES & RESPONSIBILITIES

- Complete social assessments and histories as needed
- Assist service users to identify and prioritize goals and areas of development that support wellness, empowerment, and autonomy
- Collaborate with Health Access partners and other external service partners to advocate for client needs, make referrals and provide follow up to ensure that a connection has taken place
- Work as part of the intake team and assist clients who have complex mental and physical health issues and need ongoing support services
- Work closely with Nurse Practitioner–led Primary Health Care clinic and provide wraparound services to the clients attached to primary health care.
- Provide coordination of care, promote and support clients’ participation and inclusion in the community
- Provide short term supportive and crisis counseling to a broad range of individuals with mental health and substance use issues
- Build strong partnerships with various community organizations to provide optimal harm reduction services and provide safer drug use education
- Record case information, complete accurately all necessary forms and produce statistical reports
- Accompany clients to appointments as needed.
- Maintain up to date knowledge of community resources, as well as health and social issues in the community
- Participate and contribute to community engagement activities in St. James Town
- Develop and maintain cooperative relationships with partner agencies and other service providers providing programs and services for the populations served.
- Demonstrate a commitment to a healthy and safe workplace for self and others (staff, clients, families, etc.)



by taking all reasonable precautions and working in compliance with related policies, health and safety legislation and best practices
- Participate in the development, implementation, and evaluation of programs
- Participate actively in all relevant meetings, committees, special events, etc.
- Conduct outreach to targeted population

Screening – As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening. Job Type: Full-Time permanent position.

Work Location(s): 200 Wellesley St East, Toronto, ON M4X-1G3 and other TNO locations

Schedule: 35 hours per week, with flexibility to work during TNO's operating hours of 8:30 a.m. to 8:00 p.m., including occasional weekends.

Rate of Pay: $ 29.14 per hour.
