Department: Mental Health Hospital to Home The Registered Nurse through the Hospital to Home Program – Mental Health (H2H MH) will support stabilization after hospitalization and optimize recovery while being connected to other community supports. H2H MH will provide care coordination and oversight to those receiving service from the program. H2H MH services are individually tailored with each client and address the preferences and identified goals of each client.

QUALIFICATIONS

Certification/Experience/Knowledge:

- Current Certificate of Registration ~ College of Nurses of Ontario in good standing required
- Certification from the CNA in psychiatric/mental health nursing (CPMH(c)) is an asset
- Current Basic Life Support (BLS) for Health Care Providers (HCP) Certificate required (recent within the last twelve months)
- Current Gentle Persuasive Approach (GPA) Certificate an asset
- Minimum (1-2 year) experience in Mental Health and Addictions required
- Minimum (2-3 year) medical/clinical experience in an active care setting (within last 2-3 years) required
- Knowledge of psychiatric conditions, psychotropic medication, related side effects, symptom management and health teaching required
- Demonstrated ability to support clients with a variety of mental health, addictions and physical health conditions across the adult life span required
- Demonstrated knowledge and experience in implementing a Recovery Model of Care in mental health is required
- Applied approaches in health promotion, self-management, harm reduction, disease management and health monitoring practices
- Demonstrated knowledge and experience in applying recovery informed approaches like Motivational Interviewing, Harm Reduction, Crisis Intervention and Trauma Informed Care
- Knowledge of group facilitation, program planning and patient engagement
- Demonstrated knowledge of community mental health,



addictions and vocational services
- Capillary Blood Glucose Certification
- Instruct clients and their families in health education and disease prevention, educate, support around their rights and preferences around treatment and recovery
- Evaluate and chart psychotropic medication effectiveness, complications, and side effects, and arrange for required lab work, according to protocol
- Manage and provide medication to clients to integrate medication administration into clients’ individual/recovery plans
- Participate in treatment, rehabilitation, and support services
- Cultural competency with First Nation and multicultural patient population required
- Experience in development/coordination of patient discharge care plans in collaboration with inpatient and community health care providers
- Experience with virtual meetings an asset

- Must demonstrate excellent level of interpersonal skills
- Must demonstrate excellent level of written and verbal communication skills
- Ability to work both independently and collaboratively with all members of the multidisciplinary team, clients, families, regional support services and community partners required
- Ability to provide health teaching to individuals and groups required
- Must be committed to continuing education on an annual basis and demonstrate commitment to quality improvement required
- Ability to effectively assume a leadership role as a collaborative member of the health team required




- Ability to assume responsibility for patient care in a total patient care climate required
- Able to meet the physical demands required to provide nursing care to all patients required
- Ability to problem-solve and function in fast-paced environment required
- Excellent demonstrated assessment, critical thinking and intervention skills required
- Must demonstrate basic computer knowledge
- Valid class G Driver’s License and Ministry of Transportation Driver’s Abstract for extensive travel within the catchment area
- Access to a secure and reliable vehicle with sufficient insurance ($2,000,000) to allow for work-related use
- Must demonstrate commitment to ensure personal safety, safety of fellow workers, patients, volunteers, and visitors
- Ability to support and contribute to a culture of safety and prevention of adverse health events required
- Must demonstrate good attendance and work record
- Bilingualism (Advanced level English/French) is required.
