Be a vital part of outpatient care as a Nurse Practitioner in Ambulatory Programs. Manage patient wellness across various clinics while showcasing your expertise in chronic disease management. This temporary full-time position is ideal for an experienced nurse with a master’s degree in nursing and current registration as a Registered Nurse in the Extended Class.

You will work alongside a dedicated health care team supporting numerous ambulatory programs and ensuring effective care coordination for a diverse patient population. Key Responsibilities:

- Provide patient-centered care in outpatient settings
- Communicate effectively with interdisciplinary teams
- Manage transitions of care smoothly
- Support quality improvement efforts in the program
- Address the needs of patients with chronic conditions Requirements:
- Master’s degree in nursing required
- At least 3 years of nursing experience in relevant fields
- Active registration with the College of Nurses of Ontario
- Solid critical thinking and teamwork skills
- Must be fully vaccinated against COVID-19 Utilize your nursing skills to further enhance ambulatory care experiences for patients in diverse outpatient settings.
