Registered Practical Nurse – Home Health Care VHA Home HealthCare is one of Ontario’s largest not‑for‑profit home care providers. As an RPN you will deliver and coordinate home health nursing services to diverse individuals, families and groups in accordance with the College of Nurses of Ontario Practice Standards and VHA policies.

Key Responsibilities - Assess, monitor, care plan, and coordinate care for clients.

- Evaluate care plan interventions through ongoing reassessment and evaluation.
- Ensure quality of patient care by identifying and addressing factors that affect practice autonomy and delivery.
- Demonstrate professionalism, leadership, judgement and accountability in interactions with clients and families.
- Support safe client care through development of knowledge and skills in line with continuous quality improvement principles.
- Inform the nursing supervisor of client activities, concerns or changes in condition and submit appropriate documentation to the case coordinator.
- Maintain documentation standards in accordance with the College of Nurses of Ontario.
- Participate in a standby/on‑call rotation and be available for calls and responses as needed.
- Attend team meetings during stated availability.

Qualifications & Requirements - Registered and in positive standing with the College of Nurses of Ontario (CNO).

- RPN diploma from a recognised college.




- Valid First Aid or CPR certification.
- Active Ontario driver’s licence (G) and reliable vehicle.
- Effective written and verbal communication and documentation skills.
- Reliable attendance, punctuality and availability for the role.
- Commitment to lifelong learning and the application of critical thinking in client care.
- Acute Care and Medical‑Surgical nursing experience is an asset.
- Knowledge of community nursing is an asset.
- Preference for membership in a professional association (RNAO/WeRPN).

Compensation & Benefits Pay Range: $26.60 – $32.54 per visit (salary‑range placement per Collective Agreement) + $5.00 top‑up premium per in‑person visit.

Estimated annual earning potential: $70,000–$120,000. Actual earnings may vary based on availability and service volumes.
