Responsibilities

- Assess clients’ needs through interviews, observations, and documentation to develop personalized care plans.

- Connect clients with community resources such as housing, healthcare, employment, and educational services.
- Advocate on behalf of clients to ensure they receive appropriate services and support from external agencies.
- Monitor client progress regularly, adjusting intervention strategies as necessary to promote positive outcomes.
- Maintain accurate case records, documentation, and confidentiality in compliance with organizational policies.




- Collaborate with multidisciplinary teams including healthcare providers, educators, and legal professionals to coordinate comprehensive care.

Requirements

- Bachelor’s degree in Social Work, Psychology, Sociology or a related field; Master’s degree preferred.
- Valid licensure or certification as required by local regulations for practicing social work.
- Proven experience working with diverse populations in community or clinical settings.
- Excellent communication skills with the ability to listen actively and convey information clearly.
- Strong organizational skills with attention to detail for case management and documentation.
- Ability to handle sensitive situations with empathy, professionalism, and cultural competence.
- Knowledge of local social services resources and referral processes is highly desirable.
