Job Summary & Requirements Reporting to the Manager, Addictions Services, the SUDICP System Navigator provides information, support, and service navigation to individuals living with Opioid Use Disorder and/or Alcohol Use Disorder, their families/supports, and health service providers. The Navigator delivers high-quality, client-centred information on internal RVH and community-based mental health and addictions resources by phone and in person and supports service coordination for individuals with complex needs.

Qualifications

Education:

- Degree in Social Work (BSW) or equivalent; must be a regulated health professional in valuable standing with their college (e.g., OCSWSSW)

Experience:

- Minimum 5 years of recent, relevant clinical experience working with vulnerable individuals (e.g., serious mental health conditions, homelessness, opioid/alcohol use disorders, and/or involvement with the justice system)

- Demonstrated ability to complete screening/assessment, apply problem-solving techniques, and coordinate services for complex needs

Competencies:

- Strong communication and interpersonal skills; ability to build rapport with individuals and families in a respectful, non-judgmental manner.

- Knowledge of of mental health and addictions services and community resources; ability to maintain current resource knowledge.

- Ability to assess needs, triage, and navigate complex systems; advocacy skills within health and social service settings.

- Ability to work collaboratively with an interdisciplinary team and external partners.

- Proficiency with documentation and standard office software (e.g., Microsoft Office)



and comfort with electronic record-keeping

Other:

- Ability to work a rotating schedule, including weekends, as required

- Comfort providing services by phone and in person

Responsibilities

- Provide information on mental health and addictions resources and services (internal RVH and community-based) by telephone and in person.

- Respond to individuals and/or family/support persons by phone or walk-in to provide navigation, information, and short-term support tailored to their needs.

- Use active listening, screening/assessment, and problem-solving techniques to identify needs, options, and next steps.

- Act as a liaison to connect individuals and families with appropriate services and supports, including facilitating warm hand-offs and follow-up as needed.

- Provide consultation to internal and external partners to support service planning for individuals with complex needs.

- Maintain current knowledge of community resources and internal RVH programs, criteria, and pathways relevant to mental health and addictions.

- Facilitate group sessions and education/support activities as required.

- Maintain accurate documentation, file management, and data entry in a comprehensive and timely manner.

- Complete required reporting (e.g., monthly/annual reports) and support program monitoring and evaluation activities as assigned.
