Job Summary

We are seeking a dynamic and dedicated Team Lead for the Chronic Disease Management and Prevention Diabetes Education Program (DEP). In this pivotal role, you will oversee the delivery of comprehensive diabetes education and chronic disease management services, empowering individuals to achieve better health outcomes. Your leadership will drive program excellence, foster collaborative teamwork, and ensure that clients receive personalized, evidence-based care.

Responsibilities

- Lead and coordinate the daily operations of the Diabetes Education Program, ensuring high-quality service delivery aligned with best practices.
- Supervise and support a team of healthcare educators, providing mentorship, training, and performance feedback to promote professional growth.
- Develop and implement program protocols, educational materials, and outreach initiatives tailored to diverse populations, including individuals with disabilities.
- Facilitate engaging teaching sessions, workshops, and public speaking engagements to educate participants about diabetes management and prevention.
- Collect, analyze,



and interpret data related to program outcomes to inform continuous improvement efforts.
- Collaborate with healthcare providers, community organizations, and stakeholders to expand program reach and effectiveness.
- Ensure compliance with all relevant regulations, standards, and documentation requirements for quality assurance.

Skills

- Proven experience in teaching or health education with a focus on chronic disease management or diabetes.
- Strong leadership skills with the ability to motivate and guide a multidisciplinary team.
- Excellent public speaking abilities to effectively communicate complex health information to diverse audiences.
- Skilled in data collection methods for tracking program metrics and evaluating success.
- Experience working with individuals with disabilities to adapt educational approaches accordingly.
- Exceptional organizational skills with attention to detail in documentation and reporting.
- Ability to foster a collaborative environment that encourages innovation and continuous learning.
