Patient Care Coordinator (Registered Practical Nurse)
We are seeking a Registered Practical Nurse (RPN) to join our growing multidisciplinary healthcare team as a Patient Care Coordinator.
This is a non-traditional nursing role focused on patient education, treatment planning, care coordination, healthcare products, and patient experience.
Responsibilities:
- Meet with new patients and review intake forms
- Discuss patient concerns, goals, and healthcare needs
- Educate patients regarding available services and treatment options
- Recommend appropriate healthcare products and support programs
- Coordinate treatment plans and appointment scheduling
- Track product recommendations, orders, claims, estimates, payments, and renewals
- Conduct Treatment Plan Reviews and follow-up consultations
- Ensure patients remain engaged and compliant with their care plans
- Maintain accurate documentation and coordination records
Ideal Candidate:
- Excellent communication and interpersonal skills
- Strong organizational abilities
- Comfortable educating patients and making recommendations
- Ability to build trust and rapport with patients
- Comfortable working in a structured, KPI-driven environment
- Experience in healthcare administration, patient education, or case management is considered an asset
This role is ideal for an RPN who enjoys working directly with patients, coordinating care, and helping individuals navigate their healthcare journey within a collaborative multidisciplinary clinic workplace.
Job Types: Full-time, Part-time, Permanent
Pay: $24.00-$30.00 per hour
Expected hours: 33.0 – 37.0 per week
