WHAT YOU’LL DO:
Reporting to the Clinical Lead, Virtual Care, the role of the nurse practitioner is to:
- Assess and evaluate acute and chronic health conditions for patients with an emphasis on assessment, diagnosis, treatment,
follow-up, counselling, screening, education, and referral. Provide health education and counselling focused on health promotion and disease prevention.
- Improve patient outcomes along the continuum of care through the utilization and application of up-to-date evidence-based guidelines, interdisciplinary collaboration, and participation in research.
- Work with the Clinical Lead and Virtual Care team in developing specific policies and ongoing education to take action on patient virtual care needs.
- Assist in the development and evaluation of protocols and quality improvement to provide optimal standardized care to clients.
- Provide leadership to promote program development, process improvements, and the enhancement of professional standards within the clinic or virtual care platform.
- Act as a clinical resource for nurses and other health care professionals.
- Work collaboratively with patients, physicians, and other members of the interdisciplinary team to develop a plan of care.
- Articulate and demonstrate the integration of the Primary Health Care Nurse Practitioner (NP-PHC) role through research, education, and consultation.
- Additional duties needed as per the scope of the nurse practitioner practice and the needs of the patient population and organization.
WHAT YOU’LL BRING:
- Registration without restrictions as an NP-PHC through your provincial college (Ontario mandatory, multiple licensures are preferred).
- At least three to five years as a nurse practitioner providing care in a virtual and/or in-person clinic.
- Experience in urgent care is an asset.
- Sound clinical judgment and solid health assessment and diagnostic skills.
- Demonstrated ability to work independently and as a member of a multi-disciplinary team.
- Superior and effective communication, interpersonal, organizational, and administrative skills, including computer literacy skills specific to job requirements.
- Willingness to teach/work with other professional disciplines.
- Candidate should be able/willing to work any day of the week when hours have been determined and open to different shifts.
The salary range reflects the expected compensation for this position. The final salary offered will be determined based on a holistic assessment of the candidate’s experience, education, training, licensure status, and alignment with the role’s expectations.
