Job Overview

We are seeking a dedicated and experienced Primary Health Care Nurse Practitioner to join our multidisciplinary team. This role involves providing comprehensive primary care services across various patient populations. The ideal candidate will possess a solid background in patient care and case management, with a passion for delivering high-quality healthcare.

Duties

- Conduct thorough patient assessments and develop individualized care plans as per your scope of practice
- Collaborate with our multidisciplinary team when appropriate
- Maintain accurate documentation of all patient interactions in compliance with healthcare regulations.

Requirements

- Valid Nurse Practitioner license
- Experience working independently and willing to work in a multidisciplinary setting
- Strong case management skills with the ability to coordinate comprehensive patient care plans.




- Excellent communication skills for effective collaboration with patients, families, and healthcare teams.
- Ability to handle sensitive situations compassionately and professionally.

This position offers the chance to make a meaningful difference in patients' lives through compassionate and expert primary healthcare delivery across diverse clinical settings. Our clinic currently offers Physiotherapy (Pelvic Health, Vestibular, MSK), Occupational therapy, Psychotherapy and Naturopathic Medicine.
