Job Summary
This is a casual position with the possibility of part-time. Registered Practical Nurse is a member of the home care team who works under the supervision of the Registered Nurse and performs various tasks based on the client's plan of care

Client Care Management

- Performs assessments in the community ( including nursing)
- Ensures standards of care, compliance with College of Nurses standards, state/provincial regulations and standards of care
- Develops standards that ensure safe and effective services to clients and families;
- Establishes care plan, completes and maintains related records and documentation
- Ensures a high standard of customer service is delivered to clients from intake to client conversion,



case setup- up and delivery of service;
- Ensures that the level of care established for clients is protected and appropriate based on client and home assessment
- Directs the implementation of service goals and objectives
- Ensures the evaluation of client care services
- Achieves/exceeds client satisfaction, investigates issues, and follows up promptly;

Qualifications

- Registered with the College of Nurses of Ontario (CNO) in good standing
- Experience in doing assessments in Home and Community Care ( 1-2) years
- Current BCLS certification (Level C)
- Demonstrated clinical assessment and patient/family teaching skills
- Proven excellent interpersonal skills and effective oral/written communication skills
- Demonstrated excellent organizational, time management, prioritization problem-solving, and conflict-resolution skills
- Demonstrated planning and coordination abilities
- Proven ability to work effectively as part of a team
- Demonstrated ability to work with minimal supervision

Requirements:

- Clear & updated Vulnerable Sector Check (within the last six months)
- Valid Driver’s License & reliable car
- Up-to-date TB test within the last four years
- High level of customer service experience
- Proven interpersonal, organizational, written, and verbal communication skills.
- Ability to work independently in a fast-paced, diverse, and multicultural environment.
- Demonstrated commitment to providing the highest level of satisfaction in client and staff service care

Schedule:

- Ability to see clients when needed. The ideal candidate is someone who has a part-time job and is able or willing to work casually
