This position is under the direct supervision of the North Lodge Program Manager, and overall supervision of the Executive Director. The Intake Coordinator will be responsible for the intakes and assessments for the clients to the North Lodge residential programs.
Responsibilities:
- First point of contact to community requests for intake, assessment and referrals to the North Lodge residential programs;
- Maintain waitlists for programs and ensure communication regularly with clients on waitlist;
- Ensuring written intakes/assessment reports are completed and presented to treatment team in a timely manner;
- Ensure communication avenues and linkages are developed to facilitate client access to appropriate services;
- Ensure clients are familiar with all policies, protocols and processes of the programs;
- To participate in internal and external outreach;
- To attend and participate in staff and team meetings as required;
- To coordinate all Intake Review meetings with the appropriate staff;
- Assist in designing and developing culturally appropriate approaches to intake, assessment and case management processes;
- To provide support to the teams in the management of communication, information, program statistics and data uploads;
- Liaise with other agencies, maintaining a network with area service providers;
- To provide appropriate referrals ensuring a continuum of care; and
- Other duties as required.
Qualifications:
- To possess a post-secondary diploma or Bachelor’s degree from a recognized university;
- Experience working with Indigenous communities;
- Interview and assessment skills;
- Have or willing to obtain CPR/First Aid, NVCI and other trainings as required;
- Be able to work independently and team oriented;
- Must provide a explicit Vulnerable Sector’s Check;
- Must have a valid driver’s license and reliable transportation; and
- Be a role model in the community.
Pay: $55,000.00-$60,320.00 per year
