Elevate and Synaptic are nonprofit organizations and leaders in neuro rehabilitation and complex care. We provide comprehensive, exercise-based rehabilitation for individuals with neurological conditions and other chronic or complex diagnoses. Elevate Movement works alongside Synaptic to expand access to innovative, interdisciplinary care that bridges clinical rehabilitation and real-world function.

Your Role as an Occupational Therapist

As an Occupational Therapist, you will support both the Synaptic and Elevate teams and play a key role in helping clients regain independence and participate fully in their daily lives. You’ll work within an interdisciplinary, community-based model of care that integrates physical, cognitive, psychological, and environmental factors.

Key Responsibilities:

- Deliver disability management services within a community-based,



interdisciplinary care model
- Conduct comprehensive assessments, including:
- Musculoskeletal
- Cognitive
- Psychosocial
- Functional and vocational assessments
- Perform home safety and home modification assessments to support safe community living
- Design and implement individualized Occupational Therapy treatment plans using evidence-based best practices and a client-centered approach
- Support physical, cognitive, and psychological recovery through functional, goal-oriented interventions
- Liaise with physicians, rehabilitation professionals, employers, and insurance providers
- Complete worksite-based services such as:
- Job site visits
- Ergonomic assessments
- Return-to-work planning and monitoring
- Prepare clear, professional reports with recommendations based on assessment findings

What We’re Looking For

Must-Haves:

- Degree in Occupational Therapy
- Eligibility for registration with the appropriate provincial regulatory body
- Professional liability insurance in place by start date
- Valid driver’s license and ability to travel to community and worksite locations

Nice-to-Haves (but not required):

- Experience or interest in neurological rehabilitation
- Comfort working within interdisciplinary teams
- Strong communication and documentation skills
- Curiosity, adaptability, and a growth-oriented mindset
