Make a Difference in the Lives of High-Risk Youth

The John Howard Society of Nova Scotia is seeking a dedicated and motivated Intensive Case Manager to join our Youth Horizons – Integrated Intensive Case Management (IICM) program.

This role supports high-risk, justice-involved, or gang-involved youth through individualized, trauma-informed, and culturally responsive case management. If you are passionate about helping young people overcome barriers and build stable, positive futures, we want to hear from you.

About the Role

As an Intensive Case Manager, you will provide both structured case management and hands-on community-based support. You will assess participant needs, develop care plans, coordinate services, and build strong, supportive relationships that promote accountability, growth, and long-term success.





This is a dynamic role that combines direct mentorship, advocacy, crisis response, documentation, and collaboration with community partners.

Key Responsibilities

● Conduct intake and comprehensive Bio-Psycho-Social assessments

● Develop and implement individualized case plans using principals of the Risk-Need-Responsivity (RNR) model

● Provide trauma-informed, strengths-based support and mentorship

● Assist youth with mental health, substance use, education, housing, and employment barriers

● Coordinate referrals and advocate with schools, justice partners, and service providers

● Support youth participation in pro-social, cultural, and community activities

● Facilitate restorative justice processes when appropriate

● Maintain accurate case documentation and prepare reports

● Monitor participant safety and engagement

● Participate in team meetings, supervision, and case reviews

Qualifications

● Bachelor’s degree in Social Work, Psychology, Criminology, Human Services, or related field (or equivalent experience)

● Minimum 2 years’ experience working with at-risk or justice-involved youth

● Knowledge of trauma-informed care, harm reduction,



and restorative justice principles

● Strong relationship-building and communication skills

● Excellent documentation and organizational abilities

● Ability to work independently and as part of a team

● Valid driver’s license and reliable transportation

● Ability to work flexible schedule, including evenings if necessary

TRAVEL:

● Frequent travel within the Halifax Regional Municipality area is required and occasional travel elsewhere in the province.

● Must be willing to provide accompaniment and transportation support to clients, including transporting clients in personal vehicle.

● Eligible for mileage reimbursement in accordance with JHSNS policies.
