Job DescriptionSocial WorkerLocations: Markham, Stouffville, Unionville, Thornhill, Richmond Hill.Open for Full-time and Part-TimeAs a Social Worker, you will assess, resolve, prevent or lessen the impact of psychosocial, physical and mental health related issues for clients and their families. Care will take place within a residential or institutional setting with referrals generated by a variety of sources including government agencies, employers, insurance companies or private payees. Client groups receiving treatment may include pediatrics for in home or school services and/or adults with medical/health concerns.Why you’ll love working here:Best in class training and development programsCelebration and Recognition programsPart time and Full time opportunities and flexible scheduling optionsComprehensive health and dental benefit plansCompany paid mobile devicesMileage compensationEmployee perks and exclusive offersWhat you’ll do:Conduct client assessments related to financial, accommodation, legal or mental health barriersAssist clients in setting S.M.A.R.T. goalsDesign and implement treatment plan for clients using evidence based information, best practice, and clinical expertiseWork collaboratively with a multi-disciplinary team consisting of government agencies, healthcare workers, and other community resourcesAdvocate on behalf of clients and families dealing with mental health or disabilities, chronic medical conditions, addictions, palliative and other situationsIdentify and link clients and families to available resources within the communityOffer health teaching to clients, families and caregivers as it relates to client’s medical conditionEvaluate the effectiveness of interventions, client response,



and progress toward goal achievement or for need to alter goalsCommunicate with the Health Care team regarding findings, treatments and progressMaintain professional client records related to client encounters and plans in compliance with ParaMed’s policy and the standards applied by the regulatory body or college.Maintain the knowledge, skills and attitudes to provide safe, efficient, effective service in all areas of practice by keeping abreast of changes in practice setting that affect scope of practiceWhat you bring:Degree in Social Work combined with registration /licensing with provincial regulatory collegeMinimum of 2 years’ experience as a practicing Social Worker preferably in a community settingWorking knowledge of applicable legislation including Mental Health Act, Substitute Decisions Act, Healthcare Consent Act and othersSuperior oral and written communication skills to effectively interact with clients and families possessing varying abilities for understandingEffective skills in critical thinking and problem solvingKnowledge of applicable community resources available for clients and familiesCurrent certification (within the past 12 months) of First Aid, CPR completed within CanadaAble to provide a current, clear Vulnerable Position Screening (VPS)We are committed to providing an accessible and inclusive hiring process.
