Responsibilities

- Complete comprehensive client assessments, develop care plans, and implement and evaluate care based on best evidence plus each client’s unique needs.




- Act as the point‑of‑care lead for primary provider caseloads, collaborating with other care team members and support functions.
- Support clients through care transitions by connecting them with community support services.

Requirements

- Current Certificate of Competence with the College of Nurses of Ontario.
- Availability to work days, evenings, nights, and every other weekend for 12‑hour shifts.
- Updated immunization records, a clear criminal background check, and a vulnerable sector check required upon hire.
- Willingness to attain required SE Health certifications.
- Current CPR certificate required.
- Physical demands include sitting, standing, walking, bending, kneeling/squatting, reaching chest and waist level, and various hand grips.
