Take the next step in your nursing career as a Mental Health and Substance Use Outreach Nurse at Vancouver Coastal Health (VCH). This role focuses on providing inclusive care for clients navigating mental health challenges and substance use.
Working with the Community Outreach Team, you will use crisis intervention skills and conduct assessments to address client needs. Collaborate closely with a dedicated team to devise integrated care plans and facilitate clients’ transitions to appropriate services while maintaining high standards of nursing practice.
Key Responsibilities:
• Perform comprehensive mental and physical health assessments
• Provide counseling and crisis intervention support
• Assist in the development of integrated care plans




• Help clients access essential healthcare services
• Collaborate in interdisciplinary meetings and case reviews
Requirements:
• Registered Nurse or Psychiatric Nurse accreditation
• Two years relevant experience in mental health preferred
• Must possess a valid BC Driver's License
• Familiarity with substance use treatment methods
• Exceptional problem-solving and communication skills
