Senior Support Coordinator – Homewood Health Centre

What You’ll Be Doing

- Provide support coordination for evidence‑based, complexities within the senior population with serious mental health and concurrent disorders
- Conduct comprehensive patient assessments
- Provide support coordination within a recovery framework to include treatment and community supports with both formal and informal networks
- Ensure clients are supported in community environments, with engagement in community locations at a minimum of 75% of the service period

What We’re Looking For

- Bachelor’s in Psychology, Social Work or other related field
- Valid registration with the relevant college/governing body in Ontario
- Knowledge of older adult population needs and concurrent mental health disorders
- 1 – 3 years’ experience in concurrent mental health and knowledge of the complexities within the senior population
- Valid driver’s license, business insurance and personal vehicle

Hours of Work

Permanent Full Time, 35 hours per week
