Duties
Administer medications and treatments accurately following physician orders and established protocols
Conduct patient assessments, monitor vital signs, and document health status using medical terminology and electronic health records systems such as Epic or Cerner
Assist with procedures including phlebotomy, pain management, and laboratory specimen collection
Provide specialized care for patients with dementia, seniors requiring assisted living support, or those receiving hospice care
Support case management activities by coordinating patient services and communicating with multidisciplinary teams
Educate patients and families on health conditions, medication use, and self-care strategies




Maintain compliance with healthcare regulations and ensure safety protocols are rigorously followed

Skills
Proven experience in nursing home, hospital, or acute care settings with a strong background in patient care and medical procedures
Skilled in medication administration, medical terminology, phlebotomy, and laboratory procedures
Familiarity with electronic health record systems such as Epic or Cerner for documentation and order entry
Experience working with diverse populations including pediatrics, seniors, dementia patients, and hospice clients
Knowledge of assisted living practices, home care protocols, pain management techniques, and case management processes
Excellent communication skills to collaborate effectively with healthcare teams and educate patients’ families
Ability to adapt quickly to different clinical settings while maintaining high standards of care
