As a Community Health Worker at UHN, you will collaborate with peer workers and community organizations to address the needs of clients. Your work supports those facing social inequities with a holistic approach to patient care. Engage with multidisciplinary teams to enhance client well-being and stability while advocating for their needs.
Key Responsibilities:
• Complete social issue assessments for patient support
• Educate clients and families about resources
• Facilitate engagement in community health services
• Assist clients in accessing financial support resources
• Advocate for clients’ rights and help with applications
Requirements:
• Bachelor’s degree in health or social work
• Minimum 2 years of relevant experience
• Robust knowledge of community resources
• Experience in supportive housing settings
• Excellent communication and interpersonal skills
